But what if it was never a yeast infection in the first place?

What is cytolytic vaginosis?

Cytolytic vaginosis (CV) is an overgrowth of Lactobacillus bacteria in the vagina. Yes, the same "good" bacteria that every probiotic commercial celebrates. In CV, those helpful organisms multiply beyond what the vaginal ecosystem can handle, creating an environment so acidic that it starts breaking down the vaginal lining itself. Think of it as the opposite problem from bacterial vaginosis (BV), but just as disruptive to daily life.

CV is not an infection in the traditional sense. There's no invading pathogen, no sexually transmitted organism, no fungal colony setting up camp. It's an imbalance of bacteria that are supposed to be there, which is part of why it's so routinely misdiagnosed as a yeast infection. In fact, the symptoms overlap so closely that many women end up cycling through antifungal treatments for months or even years, treating a condition they don't actually have. According to a Rescripted survey of over 2,100 women, among those who dealt with recurring vaginal symptoms, 15% said they kept treating it as a yeast infection, but nothing worked, and they still don't know what it actually was.

CV remains underdiagnosed and underresearched, a gap that reflects a broader pattern in women's health. A 1991 case series first described by Cibley and Cibley proposed the condition as a distinct clinical entity, and more than three decades later, it still hasn't made it into many standard clinical guidelines, partly because it has real pushback in the literature.

A 2020 review in Current Infectious Disease Reports argued the diagnostic criteria don't reliably rule out other causes, and that inconsistent sampling methods across studies leave the core evidence shakier than it should be. That's a fair critique, and worth knowing before you bring this to a provider. It's also exactly why so many of them have never heard of it, why it's not sitting next to BV and candidiasis in a standard guideline, and why women end up doing the research themselves before anyone in an exam room takes it seriously. The disagreement is real, but so are the symptoms.

Cytolytic vaginosis symptoms

CV symptoms mimic yeast infection symptoms so closely that self-diagnosis, and even clinician diagnosis, frequently gets it wrong. The overlap is nearly one-to-one, which is exactly why so many women end up stuck on the antifungal treadmill.

Symptoms of cytolytic vaginosis include:

  • Vulvar itching and burning

  • White or grayish, chunky discharge

  • Pain during sex

  • External dysuria (burning when urine contacts the vulva, not deep urinary tract pain)

  • Symptoms that worsen in the luteal phase — the second half of the menstrual cycle, after ovulation

That luteal phase connection is a key differentiator. During this phase, rising progesterone speeds up the turnover of vaginal epithelial cells, releasing more of the glycogen already stored there, and that glycogen is exactly what Lactobacillus feeds on. If symptoms predictably flare in the week or two before a period and ease up during or just after menstruation, that pattern is worth sharing with a provider.

One thing CV does not typically involve is the classic "yeasty" or bread-like odor typically associated with candida infections.

The discharge associated with CV is white, thick, and cottage-cheese-like in appearance, which is exactly why it gets mistaken for yeast. The mechanism behind it, though, is different. Excessive Lactobacillus activity causes lysis (breakdown) of vaginal epithelial cells, and the cellular debris produced by that breakdown creates the characteristic discharge. The discharge itself isn't the enemy. It's a visible sign of what's happening at the cellular level, and it's actually one of the most useful clues for diagnosis. Understanding what different types of vaginal discharge look like and what they signal can make it easier to identify when something is off, and to communicate it clearly to a provider.

Cytolytic vaginosis smell

CV typically produces little to no odor, or a mildly acidic smell, a key difference from BV's hallmark fishy odor. Vaginal pH also runs low, often below 4.0 and at or past the acidic end of the normal range, the opposite of BV's alkaline shift. A strong or unpleasant odor points away from CV and toward something else worth ruling out — BV, UTIs, and other vaginal conditions can produce overlapping symptoms, so odor, pH, and discharge type together are often the first real clue to what's actually going on.

What causes cytolytic vaginosis?

The root cause of CV is an overgrowth of Lactobacillus species, particularly Lactobacillus crispatus, that tips the vaginal ecosystem out of balance. Several factors can contribute:

  • Long-term or high-dose probiotic use, especially Lactobacillus-heavy formulas. The irony is real: the supplement meant to support vaginal health can actually feed the overgrowth.

  • Estrogen fluctuations across the menstrual cycle, which affect vaginal glycogen levels.

  • A diet high in refined sugars and simple carbohydrates, which can increase glycogen availability in vaginal tissue.

  • A naturally high-glycogen vaginal environment, since glycogen is the primary fuel source for Lactobacillus.

The body is essentially doing something it thinks is helpful, promoting the growth of protective bacteria. It's just doing too much of it. 

Can birth control cause cytolytic vaginosis?

Hormonal birth control can be a contributing factor, particularly formulations that raise estrogen levels. Higher estrogen increases glycogen production in vaginal tissue, which feeds Lactobacillus overgrowth. This doesn't mean everyone on hormonal contraception will develop CV, but tracking symptoms in relation to pill type, hormonal formulation, or cycle phase can create useful data points to bring to appointments.  

Cytolytic vaginosis test: how is it actually diagnosed?

Diagnosis is where so many women get stuck, as CV cannot be diagnosed by a standard yeast culture or BV swab. Those tests are designed to identify specific pathogens, and since CV doesn't involve a pathogen, they'll come back negative, which often leads to the frustrating conclusion that "everything looks normal."

Proper diagnosis requires wet mount microscopy of vaginal discharge. Under the microscope, a clinician looks for characteristic findings: abundant Lactobacillus morphotypes, lysed (broken-down) epithelial cells, bare nuclei from destroyed cells, and the absence of clue cells (which would indicate BV) and yeast hyphae. The original 1991 paper that first described CV, published in the American Journal of Obstetrics & Gynecology, laid out these diagnostic criteria and noted that CV is frequently misidentified due to symptom overlap with vulvovaginal candidiasis. Vaginal pH testing is another important tool. CV typically shows a pH below 4.0 — significantly more acidic than the normal range, and the opposite of what's seen in BV.

At a provider visit, ask for wet mount microscopy, pH testing, and simultaneous ruling out of yeast and BV. Many general practitioners may not be familiar with CV, so a gynecologist or vulvovaginal specialist may be the better route. According to the same Rescripted survey, nearly 1 in 10 respondents said they figured out something was off on their own and had to push their provider to look deeper. Advocating for the right test shouldn't have to be the patient's job, but right now, it often is.

Cytolytic vaginosis treatment

Antifungal treatments like Monistat or fluconazole will not help CV, and they may actually make things worse by further disrupting the vaginal ecosystem. This is the clearest reason why correct diagnosis matters so much.

The treatment approach for CV centers on reducing Lactobacillus overgrowth and gently raising vaginal pH back into a balanced range.

How to treat cytolytic vaginosis (and what to avoid)

The core goal is raising vaginal pH enough to calm the Lactobacillus overgrowth without overcorrecting into alkaline territory.

What to avoid during a flare:

  • Antifungal medications

  • Lactobacillus-heavy probiotics

  • Diets high in refined sugar and simple carbohydrates

  • Scented soaps, douches, or intimate washes

Lifestyle adjustments that support management include wearing loose, breathable cotton underwear, reducing dietary refined sugar, and tracking symptoms against the menstrual cycle to anticipate luteal phase flares.

Cytolytic vaginosis baking soda

Sodium bicarbonate (baking soda) sitz baths are the most commonly discussed home treatment for CV. The protocol referenced in clinical literature involves dissolving two to four tablespoons of baking soda in a shallow bath of warm water and soaking for 15 to 20 minutes, a few times per week during symptom flares. Some providers also discuss diluted vaginal rinses, though this should always be guided by a clinician.

The mechanism is straightforward: baking soda is alkaline, so it gently raises vaginal pH, creating a less hospitable environment for excessive Lactobacillus growth. Sodium bicarbonate sitz baths have been referenced as a standard management approach in the clinical literature on CV since the condition was first formally described by Cibley and Cibley, with more recent research continuing to explore bicarbonate-based formulations for symptom management.

This is a symptom management tool, not a cure. The vaginal ecosystem is delicate, and overcorrecting in the other direction (too alkaline) creates its own set of problems, including increased susceptibility to BV. Always discuss this approach with a provider before starting.

Cytolytic vaginosis boric acid

Boric acid is a popular search term in the vaginal health space, but for CV, it's likely the wrong move. Boric acid works by lowering vaginal pH, which is exactly what you'd want for BV or recurrent yeast infections, but CV already involves a pH that's too low. 

Self-treating with boric acid without a confirmed diagnosis risks making symptoms worse, and as the risks and limitations of boric acid make clear, using it without testing can delay proper care, especially if symptoms are caused by something else entirely. Always get a full diagnostic workup before choosing a treatment path.

Cytolytic vaginosis probiotics

Standard Lactobacillus probiotics may exacerbate CV, since the condition is already defined by Lactobacillus overgrowth. During an active flare, most providers recommend pausing Lactobacillus-containing supplements entirely.

After symptoms resolve, careful reintroduction may be appropriate, but strain selection matters. Not all Lactobacillus strains behave the same way in the vaginal microbiome, and this is a conversation worth having with a gynecologist or microbiome specialist. The broader takeaway is that probiotics are not universally helpful for all vaginal conditions, and the blanket "just take a probiotic" advice needs significantly more context than it usually gets.

How to cure cytolytic vaginosis: managing it long-term

CV can recur, especially if hormonal or dietary triggers remain present. Framing this as a condition to manage rather than a one-time fix helps set realistic expectations.

Long-term strategies include ongoing pH awareness (some women use at-home pH strips to monitor trends), dietary adjustments to minimize glycogen-promoting foods during vulnerable cycle phases, menstrual cycle tracking to anticipate luteal phase flares, and regular check-ins with a provider who understands vaginal microbiome health.

Living with a recurring, underdiagnosed condition is genuinely exhausting. The emotional toll of being told "it's just a yeast infection" when you know something else is going on deserves validation, not dismissal. With the right diagnosis and a tailored management plan, most people see significant improvement. Finding a provider who takes vaginal microbiome health seriously is part of that long game.

Why cytolytic vaginosis keeps getting missed, and what you can do about it

CV is not widely taught in medical schools, and it’s absent from many clinical guidelines. It's frequently dismissed or misidentified, leaving women cycling through ineffective yeast treatments, sometimes for years, while their symptoms get chalked up to "just stress" or "chronic yeast." Only 4% of Rescripted respondents with recurring vaginal symptoms said they were believed and correctly diagnosed quickly, and even they acknowledged that their experience felt like the exception.

This is a women's health research gap, and patients deserve better. Practical self-advocacy steps that help:

  • Keep a symptom journal tied to cycle phase, noting when flares start and stop

  • Request wet mount microscopy and pH testing specifically, not just a standard yeast culture

  • Bring printed information about CV to appointments if a provider is unfamiliar

  • Seek a second opinion from a gynecologist or vulvovaginal specialist if concerns are dismissed

You are not imagining this, and you’re not alone.

Finally, some answers (and a path forward)

If antifungal treatments have never fully worked for you — if the cream brings temporary relief but the symptoms always creep back — cytolytic vaginosis is absolutely worth investigating. 

The path forward starts with the right diagnosis and a provider willing to look beyond the usual suspects. Vaginal health can be complicated, but you deserve answers that actually match what your body is telling you.
